The items needed for new installations are as follows:
1--Completed new installation/reconnect/transfer forms completed to our office
2--Proof of ownership or rental/lease agreement
3--Identification such as a driver's license
4 - New Installation Fee : $850.00 Deposit: $40.00
Reconnection Fees: $100.00-Owner OR $160.00-Lease (includes deposit)
Transfer Fees: $75.00-Owner OR $135.00-Lease (includes deposit)
